What do I do if I receive damaged materials?

After inspecting the exam packets received, report any of the following conditions to the National Registry immediately:

• Torn Open

• Severely damaged

• Signs of tampering (being opened then resealed)

Take photos of any damaged exam packets and note on the Incident Report Form with as much detail and/or evidence as possible and forward to the National Registry immediately.
