What supporting documentation is needed for an Exam Accommodation Request to be approved?
Examinees requesting accommodations must provide supporting medical documentation, which may include, but is not limited to, the following:
A recommendation from a qualified professional (licensed or properly credentialed with expertise in disabilities, such as a physician, guidance counselor, disability support specialist, social worker, or psychiatrist)
Documentation of previously approved exam accommodations
Observations made by educators
Formal school documentation, such as an Individualized Education Program (IEP) or a Section 504 Plan
Results from a psycho-educational or other professional evaluation
A documented history of the individual’s diagnosis
A personal statement describing the individual’s history of exam accommodations
